Requirements for bids:

· Bidder must agree to participate in USF Program (AKA “E-rate”) for the corresponding funding year.

· Bids must include the Service Provider Identification Number (SPIN).

· By submitting a bid, bidder certifies that the bidder does have a valid (non-red light status) SPIN for the E-rate program at the time of submission. Should the district discover that the bidder is on red light status, or if the FCC classifies the bidder as on red-light status before work is performed and invoices are paid, the contract will be null and void and the applicant will have no payment obligations to the bidder.

· Bidder must agree to bill the district for only the non-discounted portion of the bill, and agrees to invoice USAC for the discounted portion (via Form 474 – Service Provider Invoice).

· Bidder will be automatically disqualified if the District determines that the bidding company has offered any employee of the District any gift or anything of value.

· Projects are contingent upon E-rate funding and final school board approval. Depending on E-rate funding, the district may choose to proceed with all or part of the projects, at the district"s discretion.

· For Priority 2 projects which involve multiple locations /campuses, Network Electronics and Cabling costs should be listed per campus, as well as summarized for the entire project.

· For Priority 2 project # 1 only, one or more on-site visits are required to district facilities before the submission deadline for the purposes of determining the exact project requirements, before submitting the proposal.  Bids from vendors who do not attend an on-site visit by the submission deadline will be disqualified. Make individual arrangements with Tom Holly, Executive Director of Technology by e-mailing tholly@midlandisd.net.
